1. Work with multi-disciplinary team to plan, manage, implement & evaluate patient care in consultation with patient and the concerned families.
2. Evaluate and monitor the patient’s progress.
3. Identify sudden or subtle changes in a patient’s medical condition.
4. Deliver regular updates to doctors, patients, and their family members.
5. Provide prescribed treatment and safe medications.
6. If necessary, respond to a medical emergency and alert the appropriate doctors.
7. Care for patient needs throughout their recovery in the ICU unit& follow IPSG.
8. Complete all necessary paperwork before transferring a patient.
9. Maintain patient records, prevent HAI & implement effective NCP.
10. Ensure upkeep of medical equipments & develop the ability to use them in case the need arise.
11. Perform approved therapeutic or diagnostic procedures based upon patients’ clinical status.
12. Evaluate patients’ vital signs and laboratory data to determine emergency intervention needs.
13. Administer blood and blood products, monitoring patients for signs and symptoms related to transfusion reactions.
14. Administer medications intravenously, by injection, orally, through gastric tubes, or by other methods.
15. Identify patients’ and families’ needs, or provide emotional support for patients and their families.
16. Monitor medical equipments and devices such as cardiac monitors, mechanical ventilators and alarms, oxygen delivery devices, transducers, and pressure lines.
17. Monitor patients’ fluid intake and output to detect emerging problems such as fluid and electrolyte imbalances.
18. Monitor patients for changes in status and indications of conditions such as sepsis or shock and institute appropriate interventions.
19. Assess patients’ pain levels, GCS & Skin as per SOP.
20. Assess patients’ psychosocial status and needs including areas such as sleep patterns, anxiety, grief, anger, and support systems.
21. Collaborate with other health care professionals to develop and revise treatment plans based on identified needs and assessment data.
22. Collect & send specimens for laboratory tests and coordinate with the treating physician for subsequent plan of care
23. Conduct pulmonary assessments to identify abnormal respiratory patterns or breathing sounds that indicate problems.
24. Provide End Of Life Care& maintain confidentiality within the legal & ethical framework.
25. Monitor for HAI & follow universal standard precautions.
26. Document patients’ medical histories and assessment findings, patients’ treatment plans, interventions, outcomes, or plan revisions
27. Identify patients who are at risk of complications due to nutritional status.
28. Prioritize nursing care for assigned critically ill patients based on assessment data and identified needs.
29. Handing over and taking charge of shift.
30. Keeping the ward clean and tidy.
31. To maintain ward inventory & Crash carts.
32. Reports and records any losses and breakage in the unit.
